Rights & Responsibilities In The Mental Health Field

LSBI believes that members should know their rights and responsibilities. Please carefully review the following information.

As a member you are entitled to:

bullet

Receive information about LSBI services, procedures, and guidelines.

bullet

Be treated with respect, courtesy, and dignity.

bullet

Participate in decisions regarding recommended plans, and length of counseling.

bullet

Participate in discussion of options regardless of cost or benefit coverage.

bullet

Have your information and records treated confidentially, in accordance with State and Federal laws

bullet

Voice complaints or appeal about services received by LSBI providers.

bullet

Make suggestions and give comments to LSBI regarding our policies and procedures.

bullet

Receive prompt, competent, and courteous services from LSBI providers and personnel.

Member's Responsibilities In The Mental Health Field

As a member you are entitled to:

bullet Get involved.  It is your responsibility to provide information in order for your provider to render and prepare evaluations and recommended plans.
bullet Participate in and follow through with your agreed upon plan when therapy was initiated.
bullet Report unexpected changes in your physical/mental/social condition to your provider.
bullet Treat LSBI personnel and providers with courtesy and respect.
bullet Refuse recommended plans any time.  However, if you do so, you are responsible for any outcome of your decision.
bullet Read your own insurance certificates/evidence of coverage/other material outlining your behavioral health coverage.
bullet If you do not understand the information, you should ask questions either to network providers, insurance company, human resources, or LSBI staff.
bullet Follow procedures for authorization, as dictated by your health plan.

Appointments

bullet

Keep scheduled appointments and be on time for your sessions.

bullet

If you have to cancel your appointment, call your provider at least 24 hours in advance.

Financial

bullet

Pay your financial responsibility at the time of service, if applicable.

bullet

Know your benefits and exclusions of your health plan.

bullet

Inform LSBI about any changes in your eligibility or coverage.

Issues and Concerns

bullet

Advise LSBI of any problems you may have and work with your representative to help resolve the issue.

bullet

Follow the procedure outlined in your health plan benefit regarding complaints and appeals.

bullet

Ask questions about the health care and services you are receiving.
