Confidentiality
In The Mental Health Field
Protection
of Privacy:
It is important that LSBI personnel
and providers protect confidential patient information from being
disclosed. LSBI staff
members follow all applicable local, state, and federal laws and
regulations with respect to maintaining confidentiality. Some of
the ways in which we work to protect your confidentiality are:

Treatment
records must be maintained in a secure area away from view of
other patients. |

Only
personnel that have a direct need to know basis have access to
a patient's record. |
 |
All
LSBI personnel sign a confidentiality statement at the time of
hire. |
 |
All
providers sign an agreement to maintain patient
confidentiality. |
 |
LSBI
will not share your information without your written
permission, unless it is required by law. In some cases, a
legal guardian must give their permission. |

LSBI
will share information as permitted by law if your safety or
the safety of others is a concern. |
 |
LSBI
follows and complies with HIPAA standards. |
Routine
Consen t
When you enroll with your health plan, you sign a form
called General Consent. Your signature at the time of enrollment
authorizes your health plan to exchange certain personal
information with LSBI,
LSBI providers,
and governmental or regulatory agencies. This limited information
enables LSBI to do its job relevant to authorization, treatment,
payment, and health care operations.

of Information in the Mental

Should you or your
provider need to release information for purposes other than
listed above, you need to sign a consent form before any
information is released.

If you are
unable to give consent for any reason, you must obtain a
consent form from a legal guardian or designated legal
representative . |

Information related
to minors is not released without the parent or legal guardian's
signature on the Release of Information form. |
Access
to Records
You can request to view and amend your records from
your provider per HIPAA guidelines. LSBI does
not use the Internet or email to communicate patient information.
Access
by
Employers
LSBI does
not provide patient information to employers.
Data
and Reports
In order to conduct
appropriate quality measures, LSBI
gathers data about services
provided to clients,
i.e., satisfaction surveys, patient follow-up, chart review, and
other sources. Individual names and other identifies are never
reported. Instead, data is always summarized.

an intake coordinator will assist you regarding your request for
services. You will be asked some questions to help us identify
your needs and select a clinician that most closely matches your
preferences for specialty and location.
